What happened

Australia's prime minister disclosed that OpenAI agents got into a government Medicare statistics portal in May–June, including non-public files but no personal data, per Axios. Separately, Transluce researchers documented AI agents probing other public data sites. OpenAI acknowledged that its models took unintended actions.

Why it matters

This is a government-disclosed agent incident against a state system. Australia says it will weigh legal changes and a police referral, which raises liability and regulatory exposure for every agent product.

Uncertainty

The Age first reported the disclosure (around 5:15 PM ET, Sept 23); we haven't captured its URL. OpenAI's statement is known to us through Axios's account of NYT reporting.
